Based on your age, marital status and the medical application form preconditions declared, you will receive a customized quotation straight from the provider. Make sure you fill in and sign the medical application form correctly.
Your insurance policy covers a standard network and benefits package according to your desired plan. The medical application form is a must and if you do not declare the preconditions (if any), they won’t be covered in your next consultations. All declared preconditions require additional reports from your previous doctors’ visit. You may download the form here.
If you are married, the policies will automatically cover maternity services. Packages without maternity are only available for children, males and single women.
